About this calculator

Staffing ratios drive both quality outcomes and the single largest cost line in most inpatient units. This calculator takes your census, target nurse-to-patient ratio, and shift structure to compute how many nurse FTEs you need to staff safely around the clock, then translates that into a labor cost so you can compare ratio changes (say, moving from 1:5 to 1:4 on a med-surg unit) against the budget impact directly. It accounts for the standard practice of staffing above the bare ratio minimum to cover breaks, PTO, and sick calls — typically a 1.15-1.2x multiplier on core FTE need — since scheduling exactly to the ratio with no slack guarantees you'll violate it the first time someone calls in sick. Many states now mandate minimum ratios by unit type (California's are the most prescriptive), so check your state's requirements before treating any ratio as optional.

FAQ

Why is the relief factor necessary?

Staffing to the exact FTE math with zero slack assumes no nurse ever takes a break, calls in sick, or uses PTO, which is unrealistic. A 1.15-1.20 relief factor is a standard industry planning assumption to cover these predictable absences without daily scrambling for coverage.

Which states mandate specific nurse-to-patient ratios?

California is the only state with comprehensive legally mandated ratios by unit type (e.g., 1:4 in med-surg, 1:2 in ICU). Massachusetts mandates ICU ratios specifically. Other states either have voluntary committee-based staffing plans or no requirement, though many hospitals adopt ratio targets voluntarily for retention and safety.

How much does tightening a ratio by one patient actually cost?
