Dozing while stopped in traffic is the item clinicians look at first regardless of total, because it indicates sleepiness severe enough to be a driving risk. Any non-zero answer there deserves a conversation with a doctor.
Sleepy is not the same as tired
Fatigue is low energy; sleepiness is propensity to fall asleep. Depression, anaemia, and thyroid problems mostly produce fatigue with a low Epworth. Apnea, narcolepsy, and chronic short sleep produce a high Epworth. Splitting the two saves a lot of wasted testing.

0 to 10 is considered normal. 11 and above indicates excessive daytime sleepiness worth investigating.
Does a high score mean sleep apnea?
Not necessarily, but it is one of the most common causes, especially alongside snoring, witnessed pauses in breathing, or morning headaches.
Is it safe to drive with a high score?
A score above 15, or any tendency to doze in traffic, is a genuine safety concern. Discuss it with a clinician before long drives.
